I've no idea. Still talking about Nome: Probably lack of emapthy because they're brazilian or something. From what I know they do care, but realistically what can they do? He does have a therapist and I don't know what else they can do for him.

Some of them can casually talk to colleagues and family, but they're just completely detached. They don't actually open up to those people, they hide parts of themselves and rarely "truly" interact with others beyond surface level shit that is expected.
If they actually opened up (i.e. admitting to all the weird shit they do in private), then in most cases someone would realize it's fucked and try to step in.
To severe addicts (internet, drugs, porn, anything), the real world is an obstacle, and "life" is when they finally get to lock themselves in their room and indulge.
(I'm aware I'm projecting, but I'm also right)
